NCIC Release Form

Fill this out if:

• you are on the team (badged or not!)
• you want to attend the next Closura (Monday of the next Kairos, at the USP main gate by 5:00 PM)

When you've filled it out and signed it, either

• Fax it to Gary Clancy at: 805-683-5625
• or, hand it to the rector.

The deadline is 4 weeks before your prison visit, or earlier, if so requested by the Rector.

NOTIFICATION TO VISITOR Form (BOP #BP-A224.022)

This is the form we used to fill out in the guard tower when we arrived each day. Fill this out per instructions from the Rector. This is for both team members and Closura visitors. This may be required in advance, with team members completing a separate copy of the form for every day of the weekend, and for the instructional reunion.

Guidelines for Closura Visitors -- All Closura Visitors should read this in advance.
