NCIC Release Form

Fill this out if:

• you are on the team (badged or not!)
• you want to attend the next Closura (Monday of the next Kairos)

When you've filled it out and signed it, either

• e-mail, Fax, USPS mail or hand-deliver it to the Rector for the upcoming Kairos weekend per his instructions.  Rector contact information is listed under the heading for the upcoming weekend.

NOTIFICATION TO VISITOR Form (BOP #BP-A224.022 - Oct 07 - partially filled in)

This is the form we used to fill out in the guard tower when we arrived each day. This copy has several parts already filled out appropriately for Kairos. This is for both team members and Closura visitors. This may be required in advance, with team members completing a separate copy of the form for every day of the weekend, and for the instructional reunion. To print: save link as... a file, then print the file on a full page (otherwise it my be reduced).

Guidelines for Closura Visitors -- All Closura Visitors should read this in advance.

"Credentials of Religious Volunteer" Form (For team members only) -- submit this with your NCIC form to the Rector for the upcoming Kairos weekend per his instructions.  Rector contact information is listed under the heading for the upcoming weekend.
